I guess most people have their home-grown mechanisms for provisioning. AFAIK, FreeRadius can authenticate against LDAP or a MySQL database, and there are frontends for, well, at least for MySQL, and I'd be very surprised if there wasn't something better than ldapsearch for LDAP...
