Whoa, Edward, so many questions :)

In the simplest scenario, you say no to IPV6 and yes to DHCP, your
provider's DHCP server gives you a lease and it works. If that doesn't
work, let the list know how you connect to your ISP, e.g. via cable modem
or ADSL, and who your provider is.
