So, you went from Ultra SCSI speeds to Fast SCSI and the problem went away?

From the dmesg below it looks you have quite a lot of SCSI devices on
your Adaptec 2940U. How long is the SCSI bus? Keep in mind that 
Ultra SCSI should use < 1.5 meter bus length. Combining external
devices (guess: the ZIP drive ?) and internal devices is not recommended.
