Hi,
Here is my configuration. The first two lines are dmesg output on my
mouse, the rest is from my xorg.conf file:
ums0: Logitech USB-PS/2 Optical Mouse, rev 2.00/11.10, addr 2, iclass 3/1
ums0: 3 buttons and Z dir.
Section "InputDevice"
Identifier "Mouse0"
Driver "mouse"
Option "CorePointer"
Option "Protocol" "Auto"
Option "Device" "/dev/sysmouse"

Are you sure you have something like
Section "InputDevice"
[...]
Option "ButtonNumber" "5"
Option "ZAxisMapping" "4 5"
in your /etc/X11/xorg.conf?
