Actually I have three of those cards, and got them to work on PAO with
just: (I.e. I do not need the 'ether 0x20).

insert logger -s D-link DE-660 inserted
insert /etc/pccard_ether $device link0
remove logger -s D-link DE-660 removed
remove /etc/pccard_ether_remove $device
card "D-Link" "DE-660L"

insert logger -s D-link DE-660 inserted
insert /etc/pccard_ether $device
remove logger -s D-link DE-660 removed
remove /etc/pccard_ether_remove $device
