Could be a terminator issue, or it could be an IRQ conflict. Do you happen to have an ethernet card that shares the same IRQ (9 perhaps) as the Adaptec controller? If that's the case, fixing the conflict can fix this problem. Other causes might be bad hardware or a bad cable.
