under ${cyrus_home}/imap you should find the file
mailboxes.db. check if the file is owned by the user "cyrus" (or
whatever user you configured).

have you carefully read the documentation in ${cyrus_source}/doc?
i am always following the installation and testing instructions step by
step and had no problems so far (except sasl, but this is
another story).

i would also try to create an account via cyradm, but see the
doc's.
