Both the book and the CD are ancient, but not to the same extent :-) 
The book describes 2.1.0R. Starting with 2.1.5R, the functionality of 
atapi.flp was merged into the standard boot.flp, so you use the same boot 
floppy whether or not you have an ATAPI CD. Anyway, 2.1.5R is very old 
too, and you should probably consider a newer version for fresh 
installations.
