Well, try simply this:

# or just Option "ButtonNumber" "3", without ZAxiaMapping

and check if
InputDevice "Mouse0" "CorePointer"
is present in Section "ServerLayout". Also, have you tried to switch
the mouse from ps/2 -> usb and vice versa?
