You add things like 'SUBDIR+=sawfish,WITH_GNOME' to the category Makefile. It's not perfect, because it can't to the readme.html stuff, but it's a start, and it's simple. The WRKDIR stuff should be handled by or the WITH_* magic that the port is using, so that the work directory becomes work.gnome or something like that...
