Which version of Apache? I have apache-1.3.9 from the ports tree and its
supplied with this in httpd.conf:

	LogFormat "%h %l %u %t \"%r\" %>s %b \"%{Referer}i\" \"%{User-Agent}i\"" combined
	LogFormat "%h %l %u %t \"%r\" %>s %b" common
	LogFormat "%{Referer}i -> %U" referer
	LogFormat "%{User-agent}i" agent
	CustomLog /var/log/httpd-access.log combined

Why not use the port? :-)
