A break-out box (with LEDs) comes in handy for these situations...

I believe ttyd* devices require Carrier Detect signal to work properly.
Try this in the host's /etc/ttys instead:
cuaa0 "/usr/libexec/getty std.9600" xterm on insecure
Dont forget to 'kill -HUP 1'
