Submitted change committed with some modifications:

- Remove the commented-lout PLIST+= line from the Makefile.
- Add some code to remove CVS dirs in share/doc/pgsql in post-install: target.
- Modify PLIST to include missing files.
- Add patch for configure so that this port would compile on 4.0. (patch-ar)
