I've narrowed it down to two test programs (the stress test
programs: creat and rw). My working theory was that it may be
related to running low of free vnodes.

Yes, I too get lots of:
tip022.log:/tmp: mount pending error: blocks 310295648 files 37638
tip023.log:/tmp: unmount pending error: blocks 3913836608 files 0
but haven't spend time on tracing the origin, as I assumed it was
harmless?
