You don't need all these enabled. Just try:
controller snd0
device sb0 at isa? port 0x220 irq 5 drq 1
device sbxvi0 at isa? drq 5
device sbmidi0 at isa? port 0x330
device mss0 at isa? port 0x530 irq 10 drq 1
to start with.
Also take a look at /usr/src/sys/i386/isa/snd/CARDS. It is for the pcm
driver (you use the ``device pcm0'' line below instead of *all* the
above), search the file for ``SB16''.
