I came across the document http://www.freebsd.org/doc/en_US.ISO8859-1/articles/mh/ during a Google search. I don't know if it's maintained, updateable, etc... but I noticed that the end of the document mentions an out-of-print book: "The best resource for MH is the O'Reilly and Associates book written by Jerry Peek." Actually, the newest versions of MH are called nmh. And there's an online version of that out-of-print book at http://www.ics.uci.edu/~mh/book/ .
