FWIW I've been having this problem as well, so far under VMWare Fusion and
ESXi. Happens when upgrading from 9.1-STABLE to 10.1-RELEASE during the last
reboot of the upgrade process (i.e., after the third freebsd-update install). I
am using UFS and have softupdates + journaling enabled.
