While this is usually an easy card to use, there have been reports of
trouble with some microcode versions, so YMMV. Also, all of my
experience is with 3.4, not 3.3, so YMMV.

Add:
device pcm0 at isa ? port ? tty irq ? drq 1 flags 0x0
to your configuration and then:
/dev/MAKEDEV snd1

Things to watch for:
1. You will get the message "pcm0 not found" at boot time. Don't
   worry. pcm0 is reserved for an ISA device. The sound card will be at
   pcm1. That's why you need to make snd1. 

2. The default audio levels seem to be zero. I had to use the mixer
   to actually hear anything.
