It means that some internal program logic got screwed up.  An I/O
request completed twice.

Without a dump, it's almost impossible to do anything with this
message.  Even with a dump, it would be difficult.  It's just barely
possible that some flaky hardware has allowed this to happen (it could
be bad program logic in recovery code, for example).
