This also happens if you subject them to far-out-of-spec overvoltage.  A
few weeks ago I accidentally used a 6V tantalum in a 24V circuit.  Oopsie.

Woke both my wife and I up rather quickly when I subjected it to the "smoke"
(or in this case, "bang!") test.
