Using the CONNECT method.

CONNECT ip_mailrelay:25 HTTP/1.0
....

It will open a conection to the ip and act has a tunnel to the 
"conversation". It's used to proxy SSL connections.

Disable CONNECT method in Squid or limit it to 443 port.
