I think I saw this same thing last night.  The device just suddenly
vanished, so I tried:

  pccardc power 0 0
  pccardc power 0 1

and it came back.  This is the SMC2632 in an IBM i-Series (2611-452)
running 4.4-RC4 + card eject patch (which otherwise seems to be
working fine).

It only happened once, but I've just started fooling with wireless
stuff this week.
