 Hi, I tried pam_abl on fresh FreeBSD 6.2 with your procedure and I
 don't have any problems - database was created after first incorrect
 logon.
 
 Please add keyword 'debug' to your pam_abl.conf and send me debug log
 after incorrect logon.
