Often in the lab I've had to low-level format each disk before the controller would post. So if disklabel is putting a label on, but not a partition table, then thats probably why the controller is wigging out on bootup. I would try using sysinstall to create a slice and label (stay away from 'dangerously dedicated mode'). Let me know if it works.
