Do you have any instrumentation suggestions? I thought that it had "gone away" because I ran for a day without seeing anything, but then it came back for long enough to kill a couple of processes, then stopped happening again. All sorts of bizarre carry-related bugs suggest themselves, but I don't fully understand your new design.
