Does this happen during `installworld'? If so, read further... During `installworld', the $PATH is set to a restricted path, and /bin/expr is not in this path. The `R' font file should normally be built during `buildworld', and installed during `installworld'. How do you build your world? Do you have anything unusual in your environment? I saw this question asked several times already, and would be interested in narrowing this down.
