Here's a suite of commandline tools for manipulating pdf's, in case you
don't want a gui:

http://multivalent.sourceforge.net/Tools/

This one uses Java, like pdfsam and pdftk.  Like pdfsam, it doesn't seem
to be in the ports tree.
