Not saying its the end all and be all, but I did have a similar problem twice before (not in FreeBSD though). The problem ended up being a IDE Controller that decided to spontaneously flake out. Unfortunately the first time the Controller was integrated into the HD, so the HD had to be replaced. The second time a simple install of a "borrowed" PCI plug in controller confirmed the problem. Not sure if this helps much, but the problem your describing is somewhat exactly (is that an oxymoron?) like the problems I experienced.
