Either of them is sufficient for FreeBSD 2.  (The hostname is
canonicalized by DNS before matching it.)

Christoph's and my problem doesn't appear _always_, only sometimes.
For me, it's about < 5 % of all my login attempts on the affected
machines.  (The target machines do all run FreeBSD 2.0.5.)  That's
what makes it weird.
