i believe that the entry outlining the uses for the refuse file (section
A.5.3.1) no longer provides reliable guidance and is in need of review. 
apparently, users can no longer selectively exclude sections of the ports
tree or their INDEX rebuilds will fail.

on the questions list we're seeing traffic related to this ever since the
following hit /usr/ports/CHANGES.

20040610:
  The following behavioural or feature changes were committed:

[big snip]

  * While building index, treat non-existent dependencies as fatal.
    Previously the error was being hidden by the stderr
    redirection.
