My EtherExpress (which works fine except for no bpf support) is configured as:
ix0 at 0x300-0x30f irq 10 maddr 0xd0000 msize 32768 in isa

According to the driver source, the size of the memory can be 16k, 32k,
48k, or 64k.  It can be located in the address range 0xC0000 to 0xEFFFF
excluding 0xEC0000 to 0xEEFFFF and 0xFC0000 to 0xFFFFFF.
