Remove the script and make it an alias or a function or something 
like that.  This will get rid of the unnecessary parent shell:

$ cat >>.profile <<eof
pppmode(){
  tt=$(tty)
  exec /usr/sbin/ppp -direct ${tt##*/}
}
eof
