Try ftp://burka.netvision.net.il/pub/xfmail/FreeBSD/xfmail.static

or, if you really want to use the shared version,

get ftp://burka.netvision.net.il/pub/xfmail/FreeBSD/libforms.so.0.7,
stick it in /usr/X11R6/lib, run `ldconfig -m /usr/X11R6/lib',
and have at it...
