Your Adaptec is going bananas.  check your termination and cabling,
something is making it really unhappy.  Also try diasabling sync transfers
or wide negotiation.

You might try getting the 2.2.5 CDROM when it's released very soon or
install a 2.2.5 beta from releng22.freebsd.org.
