Type 'show log' and look for any errors during the Authentication phase.

If that doesn't tell you anything, run 'set debug +chat' and perhaps
'+authentication' (?), dial up, then check the log again.

I assume that either you haven't set up with pap or chap and should have,
or your password or username is incorrect.
