for those new to this problem, the cause is "brain damage" in the design of
the IO APIC.  The 8259 remembers INTs that occur while masked, but the
IO APIC looses them.  APIC_LAZY avoids masking INTs when possible,
limiting (but NOT eliminating) the problem.  Scanning the mail archive
will pull up past discussion of this issue.
