Hi

With feedback from BDE, I have got the SMPng headers to a state
where they may be close to commit-ready.

The primary aim is to disentangle the sys/lock.h and sys/mutex.h
spaghetti.

Other aims are further de-spaghetti lockmgr; sort headers where
practical.
