Are you _sure_ that /usr/sbin/sendmail is the sendmail binary and not a symlink to something else? Are you sure there aren't any scripts in /usr/local/etc/rc.d/ or launched from the file /etc/rc.local which try to run smail? These are the things I would check before going any further.
