Just for the record...

I got this from Tinet, a tiny bit of tweaking to suit your setup will get it
going: 

default:
 set device /dev/modem - This is a normal symbolic link created with
FreeBSD, if describe the modem device
 set speed 115200
 disable lqr
 deny lqr
 set dial "ABORT BUSY ABORT NO\\sCARRIER TIMEOUT 5 \"\" ATE1Q0 OK-AT-OK
\\dATDT\\T TIMEOUT 40 CONNECT"
tinet:
 set phone 1891100100
 set timeout 3000
 deny pap
 accept chap
 set authname username - Change this to your username
 set authkey  password - Change this to your password
 set ifaddr 0 0
 set login ""
 set openmode active
 add 0 0 HISADDR
