My laptop is an IBM Thinkpad 350 (486/25), running 4.0-19990212-SNAP. The card itself is marketed as a generic NE2k-compatible. It identifies itself as "PCMCIA" "Ethernet Card". I can't seem to find any way to hardwire the MAC address in though, so I've been guessing offsets. All the offsets listed in other examples in that file are incorrect for this card, and I can't guess anything else that could work.
