It seems it can not read/write the disk reliably. # newfs -U /dev/ada0 /dev/ada0: 476940.0MB (976773168 sectors) block size 32768, fragment size 4096 using 762 cylinder groups of 626.09MB, 20035 blks, 80256 inodes. with soft updates super-block backups (for fsck_ffs -b #) at: 192, 1282432, 2564672, 3846912, 5129152, 6411392, 7693632, 8975872, 10258112, 11540352, 12822592, 14104832, 15387072, 16669312, 17951552, 19233792, ... ... 962962432, 964244672, 965526912, 966809152, 968091392, 969373632, 970655872, 971938112, 973220352, 974502592, 975784832 cg 0: bad magic number # dd if=/dev/zero of=/dev/ada0 bs=512 count=1 1+0 records in 1+0 records out 512 bytes transferred in 0.062203 secs (8231 bytes/sec) # dd if=/dev/ada0 bs=512 count=1 | hexdump -C 1+0 records in 1+0 records out 512 bytes transferred in 0.001043 secs (490911 bytes/sec) 00000000 00 00 00 00 00 00 00 00 00 00 00 00 00 00 00 00 |................| * 00000040 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 |................| * 00000080 ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff |................| * 000000c0 00 00 00 00 00 00 00 00 00 00 00 00 00 00 00 00 |................| * 00000100 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 |................| * 00000140 ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff |................| * 00000180 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 |................| * 00000200 # dd if=/dev/ada0 bs=512 count=1 | hexdump -C 1+0 records in 1+0 records out 512 bytes transferred in 0.001050 secs (487832 bytes/sec) 00000000 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 |................| * 00000100 00 00 00 00 00 00 00 00 00 00 00 00 00 00 00 00 |................| * 00000140 ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff |................| * 00000180 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 a5 |................| * 00000200
