I have the exact same problem *have had it since 2.0R* and I still
have it now with a new MB and disk controller. 

I now have a AHA-3940

I only have two CD''s, but it occurs on /dev/cd1a (the last one) cd0a 
works fine.

Kinda glad to see someone else has the problem... what can we do to 
help find the real problem?
