What does the version in your /usr/src/sys/dev/ata/ata-all.c look like?
(If in doubt, just use less to read that file, look round line #28.)

Mine is:

$FreeBSD: src/sys/dev/ata/ata-all.c,v 1.50.2.37 2002/06/19 08:30:48 sos

Versions 1.5.2.30 up to and including 1.5.2.36 are broken.

1.5.2.37 is fixed.

Is your hardware OK? Cable length no more than 45 cm, 80-ribbon cable
for UDMA66 or faster modes? Slave at the middle and master drive at the
end plug? Jumpers on drive correct? Any other ad stuff in the logs?
