 I've only seen this for two reasons:
 
 . your TERM variable is wrong (e.g. your .login or .cshrc foolishly
   clobbers TERM to cons25, even if you are running an xterm), or
 
 . over the network.  I think the latter problem is a known one, something
   with the (n?)curses library.
