Add:

#include <X11/Xmd.h> 

to the top of the file.  This should have been there?

Which version of XFree86 is this?  

Here on a 4.1.99 beta, Xmd.h gets included through Xlib.h via X.h.  Fxtv
should still include Xmd.h, but I'm curiousl.
