At the boot prompt (the one that has the "to boot SCSI device" stuff), type -c to configure, and make sure that the mouse is in the active part, and is not inactivated (use visual mode for best results). I had this happen to me when I was trying to install a PS/2 mouse on a 2.1.5 machine recently, and this was the fix.
