Actually it's very similar. use pnpinfo to grab the device ID from the modem, then plug that into /sys/i386/isa/sio.c, rebuild & reinstall kernel, and enjoy. If you're having trouble finding the spot look for Supra1201. It's around line 2100 somewhere.
