Are you aware of the Open Sound System from 4fron-tech. It costs $30 and offers MIDI, Synth, mmap() and a wide range of cards. I had continuous problems that their -current modules were out of date and wouldn't load, hence I don't use it for now, but it looks fair for a release user (they have a FreeBSD-3.0 binary).
