Do you happen to be using ET hardware? (www.etinc.com) I'm real curious, I've heard of related problems with exact same symptoms in conjunction with this hardware. I'm not sure at this time as to whether the problem is related to their driver, and it would be wrong to draw any conclusions. But your answer to this question would be a good thing to know.
