It still occur (at any of my freebsd boxes, even headless ones). What I did in the last hour, was to follow the suggestion from George=20 (Comment 2). I've compiled and installed libpciaccess-0.13.5. This has definitely fixed the problem and the x11 vesa driver was loaded, Xorg was working.
